    This is a lesion which might be watched by a clinician but is actually quite deep in the dentin clinically Also there is no real way to seal off an interproximal lesion predictably although there are restorative technologies such as resin infiltration by DMG Icon that appear to show promise In addition to the 15 mesial R3 carious lesion the 14 distal R3 lesion was definitely extended into the outer half of the dentine After initial preparation you will see the further extent of demineralized and discoloured dentine which was also soft extending pulpally Stained or discoloured dentine is not always infected dentine On excavation with the slow speed round you can see the next two photographs exhibit the lesion in mid preparation and in final outline form after removal of all affected dentine This was assisted via use of a caries detector dye Kuraray and slow speed round bur excavation revealing the true extent of the caries The proximal wall of the 15MO was restored with the help of an Omnimatrix and shaping this wall before applying the Dentsply Triodent V3 sectional system to the 14DO a 5 5mm SuperCurve non stick matrix was placed before the medium Pink Wave Wedge Following this a yellow premolar sized V3 tensioner ring was secured interproximally with the tines straddling the Wave Wedge The contact area was burnished with a ball burnisher Following An effort was made to recreate the slight mesiolingual concavity on the proximal wall of 15 a little light stain GC Optiglaze Colour Kit Pink Orange was touched into the fissure re creation The entire restoration was completed with a single shade of GC G Aenial Posterior A2 following a thin liner of GC G Aenial Flo X A2 to achieve an outstanding chameleon effect The post operative film

    a Kiwi Icon Levi Nek Minnit Hawken 14 October 2014 Redefining a Kiwi Icon Levi Nek Minnit Hawken Post by Dr Clarence P Tam HBSc DDS Canada The skateboarder known as Levi Nek Minnit Hawken permission gained in New Zealand presented to my service for replacement of failing composite resin restorations on multiple teeth He exhibited the classic oral signs of ectodermal dysplasia essentially a hereditary condition affecting organs derived from human ectoderm namely hair follicles nails sweat glands salivary glands and teeth The resulting oligodontia and peg shaped teeth are quite confidence disabling and results in continual alveolar ridge atrophy throughout life unless restorative intervention in the form of implants are placed to microstimulate the bone and maintain bone volume and height Typically there is a high incidence of xerostomia and significantly heightened caries risk A multi disciplinary approach is best when completing full mouth rehabilitation The patient in this case desired not to change his highly recognizable appearance but rather desired replacement of existing composite resin restorations until a more definitive dual arch solution is sought I had the privilege of conservatively redefining this now world famous New Zealand smile with Voco Grandio SO A2 Amaris O2 HT

    better while she was in the process of saving for a crown This is her starting situation After I removed her old filling this is what we were left with To restore this I used micro air abrasion 27 micron aluminium oxide first to micro roughen the tooth surface before etching and bonding This increases adhesive bond strength by up to 3 times I started blocking out the cuspal 1 3rd top 1 3rd by using Clearfil PhotoOpaquer Universal Shade This is quite a start white material Cervically at the neck of the tooth I used Voco Amaris O3 shade I extended the O3 Amaris to the cuspal edge At this stage I thought the restoration still looked too grey and stark lacking warmth I decided to add some dark brown tint in this penultimate layer Cosmedent Creative Color Dark Brown Tint in the mid facial segment to increase the warmth Now we are ready for the final enamel layer The next shot shows the restoration after placement before finishing contouring or polishing You can see how the warmth of the brown stain brings this tooth into the family around it color wise Now we are ready to finish and
