    orders of the day However this has resulted in significant organisational change Some employees are exhausted from the number of changes that have occurred in their organisation post GFC and just cannot keep up with everything I remember one of my personal frustrations was just getting used to Windows 7 when Microsoft released Windows 8 with all its user interface challenges I am still on Windows 7 and have no desire to change However in an employment situation this luxury of choice does not exist and they have to rely on the support they are given during any transitional process of new IT systems implementation and therein lies the problem All too often support training and change management is absent or barely adequate at best Companies are desperate to move forward and grow as fast as possible and this often gets overlooked Lack of post implementation support and change management from a user centred point of view results in apathy anger reduced morale negative culture informal workarounds data integrity issues and increased cases of human error For those employers please please think about the end game who will be using the systems who you rely on for good data and

    can be long term The saddest issue is how organisations respond when human error takes place their response seems to be one or more of the three listed below Persecute the individual making the error this can also include not providing enough support This often drives employees self confidence and future performance down or worse still develop severe and enduring mental health issues It is also likely to precede a reduction in other employees disclosing incidents or near misses in the future if they perceive their colleague to have been treated harshly or unfairly Investigate and respond to that individual incident very symptomatic approach a bit like sticking a plaster on your head when you finally draw blood from banging it on the desk repeatedly only to resume banging it the moment the plaster is on This will likely fix the immediate problem but will do nothing to reduce human error reoccurring in the workplace It also costs money time and risks reducing workforce morale Finally do nothing Shrug the shoulders then think that this sort of thing happens and carry on Examples of this are investigative reports on drunk drinking at house parties followed by public comments such as boys or girls will be boys and you are only young once Not very forward thinking in my humble opinion The responsibility of managing human error and indeed human factors issues often gets handed over to employees Whilst having in depth knowledge of the organisation and their area of specialism know very little of how humans respond in given workplace situations Only last weekend I heard the same person saying let the engineers get on doing their job they know best along with I don t know why that engineer behaved the way he did he has 20 years experience

    looking at the human factors issues around aspects such as decision making situational awareness socio technical systems and interpersonal elements that can make or break project success During the lecture Karl drew on his own experiences that challenged project success and made similarities to other industries where human factors is better established Some of the key areas that work against the project manager appear to lie with other people and systems A few examples include the ever prevalent danger of scope creep coupled with resistance to accept cost time impacts the changing resource availability a complex project delivery framework and last minute decisions by business systems owners that come back to bite them However assuming everything in a project goes according to plan still there are chances of human error occurring later on Training testing and support on new systems are often treated as an afterthought at a time when there is little money left over to pay for these crucial services It usually comes down to the end user having to work out what to do and more importantly what not to do This is complicated further if the system was not fully delivered or not fit for purpose and many odd workarounds have been implemented To put this into a personal context just think about anything that you own at home that has a few quirks no not your partner It could be your car your PC operating system your entertainment system your food processor anything Now imagine if we did an upgrade on it but in doing so introduced a few new quirks It would take you time to figure out what they are and how it affects you A few mistakes will be made by you no doubt until you learned the new ways of operating

    some of the human error related issues we hear How do you prevent or protect against this There are a wide range of quality and risk management tools out there and it is not my place to criticise them However a lot of businesses fail to understand that human error is not just an issue of making a mistake and putting it right Understanding the conditions leading up to the event takes a lot of know how The defence industry know this because they don t want their people killed in action The aviation industry know this because they don t want their pilot s making silly mistakes with 400 paying customers sitting in the back seats And even the medical industry is getting on board with how mistakes in theatre occur as a result of fatigue or overwork All of these industries know the importance of having human error and reliability expertise to help them understand how people think act and decide their next course of action to take So with that in mind do you manage human error Do you investigate any incidents Not just those related to health and safety but also operational and back office errors Do you find that the corrective actions always seem to be the same e g training needs but the errors keep occurring What do you do with the investigation data afterwards Investigations need to be robust and the findings and actions need to be analysed It is hard to know what to analyse and what questions to ask to ensure that you have a good chance of preventing similar human errors occurring in the future Without having the foundation of knowing how humans react think and behave in your business how can you possibly prevent human error Concerning prevention a

    into the limelight for discussion As we face a future of increased complexity of organisational systems IT policy project management process etc we also have to face the fact that humans by their very nature are limited by their ability to make mistakes particularly under pressure and working in complex environments As a result if an employee is disciplined or dismissed for making a mistake the chances of someone else making a similar mistake in the same organisation in the future are significant ask any large organisation that have frequently been in the press for all the wrong reasons So I was on a flight back home the other week and got the comment So you work with human error That s to do with health and safety right Wrong Although when we think of some of the key people who have researched human error they generally focus on health and safety incidents it appears to me that it is the errors in everyday organisational operation that can also have significant damaging effects Okay no one has been injured or worse which is a good thing but the damage to the organisation s reputation can be just as severe Again it is not hard to think of some organisations that have been put through the media mill lately Already in 2014 we are seeing a spate of human errors the most eye catching for me was the estimated loss of 810 000 due to a market trader entering the wrong figures causing a 10 spike in shares in one of the world s largest banking institutions the loss occurred over a period of 30 seconds Human error is part of the human condition and it will never go away Even when we add automation we still require a human to

    as possible the company your work for want to make money too you both want to make money as easily as possible There is nothing wrong with these basic principles At the end of the day we all want food on the table for our families But there are things that can really ruin the chances of making money illness losing your job workplace accidents and not being as productive as you or your work mates originally planned So we have a basic idea of how why we work and what can happen to mess our day up This is where human factors comes in Human Factors is all about understanding how you and your workplace operate Human Factors Specialists try to ensure you are given the tools and conditions to function safely and efficiently as well as make as much money as possible Human Factors Specialists train in human psychology physiology engineering design and businesses and they have a good idea of how people operate in different high risk environments They also know what the potential mental and physical impacts that may arise as the result of someone in a large organisation or central government making a decision How could anyone possibly know how it affects me You may ask Does it surprise you to know that there are many organisations in the world that have made some very good and very bad decisions Some companies have made some incredibly bad decisions that resulted in them going bankrupt within a year or even killing someone Another couple of facts worth thinking about the majority of people go to work to do a good job your employer knows you want to do a good job too Now it is fair to say that many companies have learnt the hard way
